What We Offer
A HomeCare RN Case Manager coordinates comprehensive patient care in the home, assessing needs, developing individualized plans (POCs), and acting as a liaison between patients, families, doctors, and insurers to ensure quality, cost-effective treatment, often supervising other home health staff and providing direct skilled nursing care.
What Youll Do
• Responsible for coordinating home care services which includes establishing and maintaining the patient's plan of care. • Customer service and financial management of your assigned caseload. • Conducts admissions, recerts, ROC, and discharges for the assigned case load. • Conducts routine nursing visits as needed to assess patient needs. • Coordinate patient care between the patient, caregiver, physician, and other appropriate disciplines. • Responsible for the completion of all home health documentation.
What Were Looking For
Graduate of an accredited school of nursing required. Minimum Associate's or preferred Bachelor's degree in nursing. Current valid North Carolina nursing license required. Basic Life Support (BLS) through American Red Cross or American Heart Association required. Driver's License Class C. Minimum 1 year of home health or related RN experience required.
