Location: Honesdale, PA Schedule: 8 AM – 5 PM, Monday–Friday | 6 units daily / 30 units weekly On‑Call: 1 weekend per month + 1 evening per week Territory: Honesdale and approximately 15–25 miles surrounding the community Travel: Local travel across multiple counties required
Overview
This opportunity is ideal for a compassionate and organized Registered Nurse Case Manager who is committed to delivering exceptional hospice care. In this role, you will coordinate patient care, support families, and serve as a clinical leader within a collaborative, mission‑driven team. You'll work independently in the field while maintaining strong communication with interdisciplinary partners to ensure patients receive dignified, high‑quality end‑of‑life care.
Key Responsibilities
Manage and oversee patient care within hospice settings, ensuring individualized, patient‑centered services
Develop, implement, and evaluate comprehensive care plans
Collaborate with interdisciplinary healthcare teams, including physicians, social workers, and support staff
Provide leadership, mentorship, and guidance to nursing staff
Maintain accurate, timely clinical documentation
Communicate effectively with patients, families, and caregivers
Participate in on‑call rotation (one evening weekly + one weekend monthly)
Travel locally to patient homes and facilities across multiple counties
Required Qualifications
Education: High School Diploma or GED
Experience:
2+ years of Hospice experience
2+ years of Management or leadership experience
1+ years of experience in ANY of the following:
Home Health Care
Epic Home Health
Home Health
Licensure:
Valid Registered Nurse (RN) license in the state of Pennsylvania (PA)
Certifications (ALL required):
American Heart Association Basic Life Support (AHA BLS)
American Heart Association Advanced Cardiac Life Support (AHA ACLS)
Other Requirements:
Reliable transportation and ability to travel locally
Strong time‑management and self‑organization skills
Ability to work independently in field‑based environments
