NEW- Registered Nurse Case Manager

Job Schedule: 5x8, 08:00-16:30, Monday-Friday, 13-week contract. 

Requirements:

• Valid Registered Nurse (RN) license.
• Minimum of 2 years of hospital case management experience required
• Strong communication and organizational skills.
• Ability to work independently and as part of a team.
• Proficiency using Cerner EMR 

Job Responsibilities:

• Coordinate patient care and ensure continuity of care across various healthcare settings.
• Develop and implement individualized care plans for patients.
• Collaborate with physicians, therapists, and other healthcare professionals to optimize patient outcomes.
• Monitor patient progress and adjust care plans as necessary.
• Educate patients and their families about health management and care processes.
• Maintain accurate and up-to-date patient records.
