Job Description
NEW- Registered Nurse Case Manager
Job Schedule: 8:00 AM - 4:30 PM or 8:30 AM - 5:00 PM, Monday through Friday with rotating weekends and holidays. 13 week contract.
Requirements:
- Valid Registered Nurse (RN) license.
- Minimum of 1+ years of hospital case management experience
- Strong communication and organizational skills.
- Ability to work independently and as part of a team.
- Proficiency using EPIC EMR system
Job Responsibilities:
- Coordinate patient care and ensure continuity of care across various healthcare settings.
- Develop and implement individualized care plans for patients.
- Collaborate with physicians, therapists, and other healthcare professionals to optimize patient outcomes.
- Monitor patient progress and adjust care plans as necessary.
- Educate patients and their families about health management and care processes.
- Maintain accurate and up-to-date patient records.
