Responsibilities
- Coordinate care and plan transitions throughout the acute care patient experience.
- Collaborate with physicians, social workers, and other healthcare professionals to ensure timely patient progression and transition.
- Develop cost-effective care coordination plans meeting clinical needs.
- Monitor patient length of stay and manage resources efficiently.
- Formulate transition plans in collaboration with patients, families, and healthcare teams.
- Identify and address barriers to care progression and transition.
- Communicate effectively with financial counselors and make appropriate referrals.
- Contribute to creating cost-effective practices for optimal patient/provider experiences.
