Job Duties
Responsible for care coordination and care transitions planning throughout the acute care patient experience. Collaborate with physicians, utilization managers, medical social workers, and bedside RNs to ensure timely progression and transition of patients to the appropriate level of care. Prevent unnecessary admissions or readmissions through effective resource coordination.
Key Responsibilities
- Conduct patient initial and continued assessments, including medical, psychosocial, and financial information gathering.
- Develop comprehensive, cost-effective care coordination plans.
- Monitor length of stay and manage utilization to ensure appropriate level of care.
- Collaborate with a multidisciplinary team to resolve barriers to care progression and transition.
- Identify and address complex clinical and social situations to avoid unnecessary admissions.
- Ensure compliance with state and federal regulatory requirements for care transitions.
- Develop and maintain professional relationships with healthcare team and community agencies.
Department Operations
- Contribute to improving practices and creating cost-effective solutions.
- Actively engage in department meetings and communications.
- Provide and accept constructive feedback to enhance team performance.
