Experience:
- 5 years of RN Case Management experience
- Inpatient Acute Case Management (Hospital experience required)
Job Duties:
- Coordinate care and transition planning throughout the acute care patient experience.
- Collaborate with physicians, medical social workers, and bedside RNs to ensure timely patient progression to appropriate care levels.
- Prevent unnecessary admissions or readmissions by facilitating effective care across the continuum.
- Engage with patients, families, and interdisciplinary teams to achieve optimal health and resource utilization.
Job Accountabilities:
- Conduct patient initial and continued assessments.
- Supervise and lead healthcare teams in developing cost-effective care coordination plans.
- Monitor length of stay and expedite transition planning for patients.
- Ensure compliance with CMS and Joint Commission Transitions of Care requirements.
- Formulate and communicate transition plans with patients, families, and healthcare teams.
- Address complex clinical and social situations to avoid unnecessary admissions.
- Actively participate in department operations and contribute to process improvements.
