Job Duties

  • Coordinate care and transitions throughout the acute care patient experience.
  • Collaborate with physicians, medical social workers, and bedside RNs.
  • Ensure timely progression and transition of patients to prevent unnecessary admissions or readmissions.
  • Communicate effectively and facilitate care across the continuum.
  • Develop cost-effective care coordination plans.
  • Identify and refer quality and risk management concerns.

Job Accountabilities

  • Conduct patient assessments and develop care plans.
  • Manage utilization and ensure appropriate level of care.
  • Formulate and implement transition plans.
  • Address complex clinical and social situations to avoid unnecessary admissions.
