Job Description:

We are seeking an experienced Registered Nurse (RN) Case Manager for a 13-week contract assignment in Santa Rosa, California. This role focuses on ensuring patients receive appropriate, cost-effective, and timely care throughout their hospitalization and transition to the next level of care.

2-year acute Hospital Case Manager experience performing Care Coordination, Discharge Planning in a very fast paced environment.
Patient Ratio Experience: 1:20
Certification: BLS
