Job Description
We are seeking an experienced Registered Nurse (RN) – Case Manager / Utilization Review / CDI to provide case management and utilization review services within an acute care hospital setting.
The RN will apply clinical knowledge, assessment skills, and healthcare documentation expertise to support patient care, coordinate services, facilitate appropriate utilization of healthcare resources, and assist with discharge planning. Experience with EPIC and acute care hospital case management is required.
Key Responsibilities
Provide case management services within an acute care hospital environment.
Perform utilization review and support appropriate utilization of healthcare resources.
Coordinate patient care and services across multidisciplinary healthcare teams.
Review patient clinical information, medical records, and supporting documentation.
Utilize EPIC Electronic Medical Records (EMR) as required.
Support discharge planning and care coordination activities.
Communicate effectively with patients, healthcare providers, and multidisciplinary team members.
Maintain accurate, complete, and timely clinical documentation.
Collaborate with physicians, nurses, social workers, and other healthcare professionals to facilitate quality patient care.
Follow applicable hospital policies, procedures, and patient safety standards.
Maintain patient confidentiality and professional standards.
Perform other related nursing, case management, utilization review, and documentation duties as assigned.
Requirements
Education, Licensure & Certifications
Current New York State RN registration/license required.
Current BLS certification required and must be eligible for recertification.
Must meet all applicable professional licensing and certification requirements.
Experience & Skills
Minimum 1.5 years of recent experience within the past 2 years in an acute care facility of similar size and level required.
Previous acute care hospital case management experience required.
Experience with EPIC Electronic Medical Records preferred/required based on assignment needs.
Travel/contract nurses must have experience working in hospitals of comparable size and/or trauma centers.
Strong clinical assessment, documentation, utilization review, and care coordination skills.
Excellent interpersonal and communication skills.
Ability to work effectively with multidisciplinary healthcare teams.
Strong organizational skills and attention to detail.
