The CN leads their team and participates in, and supports performance improvement activities designed to promote and enhance quality and safety, improve efficiency in care and delivery, and evaluate the impact of patient outcomes. The CN is responsible for the care management and coordination of the patient panel (s) and acts as an advocate, educator, and resource person for this population. Applying primary care medical home concepts, the CN looks at the provider team's population health trends and takes action to improve patient outcomes.
