What You Will Do:Champion patient recovery by executing personalized care plans and adapting treatments to meet the evolving needs of those in your care.
Lead the multidisciplinary team as the primary coordinator, ensuring seamless communication between doctors, specialists, and support staff.
Advocate for safe transitions by managing comprehensive discharge planning to empower patients and families for life after the hospital.
Uphold clinical excellence by delivering evidence-based nursing care and maintaining precise documentation of the patient’s healing journey.
Licensure / Certification / Registration:
Education:
Specialized Nursing Background: 2–3 years of Registered Nursing experience in hospital settings, specifically within Cardio and Vascular departments, including hands-on clinical surgery support.
Comprehensive Care Delivery: Proven expertise in both acute hospital environments and outpatient clinic settings, supplemented by a working knowledge of hospital coding practices.
