Registered Nurse - Clinical Care Coordinator III - FPG Centralized Clinical Services, Sarasota

Job Summary:

The Clinical Care Coordinator III demonstrate the competency to provide patient care according to the age-specific population needs and assist in the management of patient care in the physician office setting under the direct supervision of a provider.

 

This position is with FPG Nurse Triage Team - will work at FPG's Centralized Access Center on University Parkway in Sarasota. 

 

The RN will take calls from established FPG patients and follow protocols for patient symptoms and offer guidance based upon strict adherence to established protocols. The goal is to provide excellent patient care and provide real time clinical guidance. This team will work closely with the Clinical Excellence and Operations team to ensure seamless patient care experiences in collaboration with providers and develop effective communication with the provider office care teams.

Required Qualifications:

- Require Basic Life Support/CPR certification; or must be obtained within thirty (30) days of hire/transfer date.
- Require graduate of an accredited school of nursing or a diploma program.
- Require a minimum of one (1) year of clinical practice experience in a medical/physician office setting.

Preferred Qualifications:

- Prefer Electronic Health Records experience.
- Prefer demonstrated ability in establishing and maintaining effective working relationships.
- Prefer strong oral and written communication skills.
- Prefer the ability to perform multiple tasks simultaneously, while maintaining proficiency and efficiency.

Mandatory Education:

AS: Associate of Science
