    Registered Nurse (RN) – Home Care

    Job Summary

    We are seeking a compassionate and experienced Registered Nurse (RN) to provide high-quality care to patients in the comfort of their homes. The RN will be responsible for assessing patient health needs, developing care plans, administering treatments, and collaborating with physicians and care teams to ensure optimal patient outcomes. The ideal candidate is patient-focused, detail-oriented, and committed to providing exceptional home healthcare services.

    Work Environment

    This position requires travel to patient homes within the assigned service area and may involve standing, lifting, and assisting patients with mobility as needed.

    Responsibilities

    • Conduct comprehensive patient assessments and develop individualized care plans.

    • Administer medications, treatments, and IV therapies as prescribed by physicians.

    • Monitor patient conditions and document clinical observations and progress notes.

    • Provide wound care, catheter care, injections, and other skilled nursing services.

    • Educate patients and families regarding disease management, medications, and home care procedures.

    • Coordinate care with physicians, therapists, caregivers, and other healthcare professionals.

    • Maintain accurate documentation in accordance with state regulations and agency policies.

    • Ensure compliance with infection control, safety standards, and HIPAA regulations.

    • Respond to changes in patient conditions and communicate concerns to the appropriate medical provider.

    Qualifications

    • Active Registered Nurse (RN) license in the State of [State].

    • Current BLS/CPR certification required.

    • Minimum 1 year of nursing experience (home health or clinical experience preferred).

    • Strong clinical assessment and critical thinking skills.

    • Excellent communication and patient education skills.

    • Ability to work independently and manage multiple patient cases.

    • Reliable transportation and valid driver’s license.

    Preferred Experience

    • Home health or community-based nursing experience.

    • Experience with IV therapy, wound care, and chronic disease management.

    • Familiarity with electronic health record (EHR) systems.
