    2 years RN experience required in an outpatient setting

    RN ICU or Operating Room experience

    M- F, weekends and holidays on occasion, some on- call.

    A challenging role supporting patients with advanced heart failure who receive mechanical circulatory support (MCS) devices. The position manages the full care pathway-from patient identification and evaluation to implantation and long-term follow-up-while coordinating multidisciplinary care and device management. Responsibilities include patient and staff education, long-term care coordination, data collection, and participation in quality improvement initiatives. The role also requires on-call coverage M-F, nights, and weekends.

    Ideally looking for RN with cardiology background.

    GENERAL SUMMARY:

    • Transplant Coordinators coordinate all aspects of care for both transplant recipients and donors.
    • A coordinator participates in medical, administrative, research and teaching duties.
    • The coordinator is an active member of the multi-disciplinary transplant team.

    EDUCATION/EXPERIENCE REQUIRED:

    • Bachelor's degree in nursing preferred.
    • Organizational and Interpersonal skills to effectively communicate with patient care personnel and relate to the patients and their families.
    • Minimum of two (2) years clinical nursing experience in an inpatient or outpatient setting.
    • Analytical ability and motivation necessary to effectively plan, deliver and implement plans of care to in acute and outpatient situations.
    • For those working as a Coordinator of Heart Transplant/with VADS: Experience in ICU or OR preferred.

    CERTIFICATIONS/LICENSURES REQUIRED:

    Current RN license in the State of Michigan.
