Registered Nurse

Key Responsibilities

1. Perform Start of Care (SOC) visits
2. Complete OASIS documentation accurately
3. Conduct routine skilled nursing visits
4. Perform recertification and discharge visits
5. Medication reconciliation
6. Patient education and preventive care
7. Coordinate home health services and referrals
8. Collaborate with providers and care team
9. Same-day documentation
10. Manage assigned caseload and perform patient assessments

1. Deliver wound care, catheter care, and other skilled nursing services

1. Educate patients and caregivers on disease processes and care plans

1. Provide medication education and management

1. Document care accurately using EMR systems

1. Collaborate with interdisciplinary care teams to ensure continuity of care

·       Meet productivity expectations and quality standards

Productivity Expectations: Expected Productivity: 30-40 points per week

Required Qualifications

1. Active Registered Nurse (RN) license

1. Home health nursing experience (required)

1. Ability to work independently in a field-based setting

1. Strong clinical, assessment, and patient education skills
