Overview
The registered nurse provides clinical leadership and direct patient care for a research program that is developing a strategy to improve brain and heart care in geopolitically defined communities (i.e., cities, neighborhoods).  The program is starting in the City of Derby, CT with a focus on hypertension care.  The RN will support and supervise community health workers; he or she will guide the community health workers in conducting participant interviews, obtaining vital signs, securing informed consent, providing health education and conducting follow-up according to protocol.  In addition to supervisory responsibilities, the registered nurse will work alongside the community health workers to enroll, consent, examine, and counsel program participants.  The nurse will manage a caseload of participants in the program to apply care algorithms, evaluate symptoms, communicate health information, and assure adherence with the program protocol as it pertains to nursing care.  The registered nurse will use clinical judgment to triage and direct program participants to the appropriate level of care. The registered nurse will communicate with program participants, families, and other healthcare providers to support continuity and coordination of care. The registered nurse will contribute to maintaining high standards for medical care, supports program quality improvement, and collaborate with the interdisciplinary team to optimize outcomes for all program participants.

Required Skills and Abilities

1. Current licensure as a Registered Nurse and current BLS or ACLS CPR certification.
2. Excellent written, oral, and interpersonal communication skills.
3. Strong organizational, clinical and critical thinking skills with the ability to work under the direction of advanced healthcare professionals.
4. Ability to work with acutely and chronically ill patients across multiple disciplines, populations, and clinical settings.
5. Proficiency in using electronic medical records and basic software such as Microsoft Office.

6. Certification in human subjects research (can be obtained after hire).


Preferred Skills and Abilities

1. Bachelor's Degree in Nursing (BSN) or higher.
2. Experience in ambulatory care, community health, case management, or care coordination.
3. Experience with EPIC or other electronic medical records systems.
4. Experience in clinical research, including implementation of protocol-directed care and other research procedures.
5. A minimum of 2 years of nursing experience.

6. Experience with REDCap or similar electronic data capture system for research.

7. Experience with or prior work with the community in the City of Derby or nearby communities in the Naugatuck Valley.

Principal Responsibilities

 

1. Performs physical assessments of patients, including triage duties and advises on treatment or course of action by having knowledge of nursing care standards and principles. 2. Develops, implements, and manages nursing care plans in collaboration with physicians. 3. Interviews patients and records medical history to establish data for treatment plan. 4. Plans and coordinates the admission and discharge of all patients. 5. Instructs patients and families in proper treatment and advises patients on health maintenance and disease prevention. 6. Performs patient care procedures by following appropriate policy and procedure. 7. Records patients’ medical histories and symptoms, operates medical machinery, administers treatment and medications, and coordinates patient follow-up and rehabilitation; assures all procedures for maintaining patient confidentiality are maintained per HIPAA guidelines. 8. Identifies education needed to assure competent delivery of patient care for oneself and others in the department. 9. May perform other duties as assigned. Required Education and Experience Graduation from accredited school of nursing. Required License(s) or Certification(s) Licensure as Registered Nurse in the State of Connecticut or Connecticut license eligible. Current CPR certification.
