About the Job
As an Ambulatory Registered Nurse in our Diabetes & Endocrinology practice, you will play a vital role in delivering specialized care to patients managing chronic endocrine conditions.
You will collaborate closely with endocrinologists, advanced practice providers, and care teams to provide evidence-based, patient-centered care, including in-office care, telehealth services, and chronic disease management support.
What You'll Do
Deliver comprehensive nursing care using the nursing process (assessment, planning, intervention, evaluation) with a focus on chronic disease management
Support patients with diabetes care, including blood glucose monitoring, medication adherence, and lifestyle management
Educate patients on:
Insulin administration and injectable therapies
Continuous glucose monitors (CGMs) and insulin pumps
Nutrition, weight management, and lifestyle modifications
Collaborate with providers to develop and implement individualized endocrine care plans
Administer medications, vaccines, and perform point-of-care testing (e.g., A1C, glucose checks)
Provide telephone triage and telehealth support, addressing patient questions and symptom management
Document care accurately in the EMR and assist with pre-visit planning
Monitor and follow up on lab results, medication adjustments, and care plan adherence
Serve as a resource and mentor to clinical staff; assist with onboarding and education
Respond to urgent clinical concerns in alignment with established protocols
