| Duties: | Reports to the Dialysis Nurse Manager/Coordinator of the Dialysis program. Maintains a collaborative working relationship with the Medical Director, Manager, Technical Leader and all unit staff. Functions include admission and ongoing assessments of patients utilizing physical and psychosocial assessment skills, diagnostic data, and medical evaluations. Evaluates effectiveness of care provided as related to short and long term patient goals. Develops a plan for patient care including actions in anticipation of individual patient needs. Implements nursing actions and medical orders to meet complex needs of patients and significant others, taking into account their age specific psychosocial and educational needs. Documents care provided and patient responses to care in appropriate forms in the patient's record. Assumes Charge Nurse responsibilities and accountability for care provided by LPNs and non-licensed technical personnel as assigned. Leadership abilities are demonstrated in the following: functioning as a professional role model, consumer relations, communication skills, problem solving, decision-making, and assistance in orienting and training personnel. Demonstrates team member standards and adheres to diversity principles and expectations including deaf and hard of hearing guidelines. PRINCIPAL DUTIES AND RESPONSIBILITIES: 1. Demonstrates ability to observe and assess patients on an ongoing basis, keeping in mind age-specific needs.
2. Demonstrates ability to respond in an emergency situation.
3. Participates in primary care nursing/case management and will provide direct patient care as necessary.
4. Performs blood-drawing procedures.
5. Maintains neat and safe work area.
6. Monitors and adjusts dialysis therapy to meet the needs of the patient, keeping in mind age-specific needs.
7. Administers and monitors medication according to policy.
8. Contributes to on-going patient education and rehabilitation. Provides modality education to patients.
9. Serves as a referral source for the patient.
10. Documents nursing care and observations, (for example, pre-post temp, pre-post assessment and completion of treatment records).
11. Demonstrates a professional approach to job.
12. Communicates with other hospital departments, outside agencies and dialysis facilities.
13. Functions as a part of an interdisciplinary team. Oversees care provided by clinical technicians.
14. Treats patients and families with respect and dignity.
15. Demonstrates flexibility in work situation.
16. Demonstrates ability to plan work activities.
17. Follows infection control policies and procedures (e.g., hand washing, face shield and gloves).
18. Follows operational activities specific to assigned unit, (e.g., call-in times, equipment and machine checks, breaks).
19. Demonstrates ability to minimize waste in assigned unit.
20. Demonstrates acceptance of constructive criticism and direction from supervisor.
21. Demonstrates problem-solving methodologies for benefit of patients/significant others, the patient care area and the nursing profession. Does so within Product of authority.
22. Carries out doctors' orders within parameter of role.
23. Demonstrates technical and task-oriented skills (e.g. cannulation, machine setups, disinfection).

| Education: | EDUCATION/EXPERIENCE REQUIRED: Graduate of state approved school of nursing. Possesses current RN licensure with the applicable State Board of Nursing. Prior to assuming charge responsibilities, must have applicable State Board of Nursing.
