Position Summary 

As a Registered Nurse (RN), you will provide comprehensive skilled nursing care to patients in their homes throughout the assigned service territory. You will assess patient needs, develop individualized plans of care, coordinate services with the interdisciplinary team, and educate patients and caregivers to promote independence and improved health outcomes. 

This role offers the opportunity to build meaningful relationships with patients while enjoying the autonomy, flexibility, and purpose that home health provides. 

Responsibilities 

  • Perform comprehensive patient assessments and develop individualized plans of care. 

  • Complete Start of Care (SOC), Re-certification, Resumption of Care (ROC), Discharge, and routine skilled nursing visits. 

  • Administer medications, treatments, injections, wound care, and other skilled nursing interventions as ordered. 

  • Monitor patient conditions and communicate significant changes to physicians and the interdisciplinary care team. 

  • Coordinate patient care with therapists, aides, physicians, and other healthcare professionals. 

  • Educate patients and caregivers regarding disease processes, medications, treatments, safety, and self-management strategies. 

  • Ensure timely and accurate clinical documentation in accordance with agency, state, and federal regulations. 

  • Participate in case conferences and collaborate to achieve optimal patient outcomes. 

  • Demonstrate accountability and ownership in achieving exceptional patient outcomes. 

  • Maintain compliance with Medicare Conditions of Participation and all agency policies. 

Qualifications 

  • Graduate of an accredited Registered Nursing program. 

  • Current Ohio Registered Nurse (RN) license in good standing. 

  • Current CPR certification. 

  • Valid driver's license, reliable transportation, and automobile insurance. 

  • Home health experience preferred but not required. 

  • OASIS experience preferred. 

  • Strong assessment, critical thinking, clinical, communication, and organizational skills. 

  • Passion for delivering exceptional care and making a meaningful impact in patients' lives.
