Intermountain Health is seeking a compassionate and patient-centered Registered Nurse for our Extended Care team within our Healthcare & Medical Services department. In this role, you will provide high-quality, evidence-based nursing care to adult and geriatric patients who require extended, rehabilitative, or long-term services following hospitalization, surgery, or chronic illness exacerbations. As a key member of an interdisciplinary care team, you will coordinate and deliver holistic care that emphasizes safety, dignity, and optimal functional outcomes for each patient.

The Registered Nurse – Extended Care will perform comprehensive assessments, develop individualized care plans, administer medications and treatments, monitor patient status, and respond promptly to changes in condition. You will collaborate closely with physicians, therapists, social workers, case managers, and support staff to ensure seamless transitions of care and to support patients and families in understanding diagnoses, treatment plans, and self-care strategies. You will also play an active role in discharge planning, patient and family education, and care coordination with community resources.

Working within Intermountain Health’s culture of excellence, compassion, and integrity, you will champion patient safety, infection prevention, and quality improvement initiatives. You will use clinical judgment and critical thinking to prioritize care, manage complex caseloads, and advocate for patient needs, while maintaining accurate and timely documentation in the electronic health record. You will participate in interdisciplinary rounds, contribute to care conferences, and support performance improvement projects aimed at enhancing patient outcomes and satisfaction.

In this position, you will supervise and delegate tasks to licensed practical nurses, nursing assistants, and other support staff in accordance with scope-of-practice regulations and organizational policies. You will provide mentorship, guidance, and feedback to ensure high standards of care and foster a supportive, team-based environment. You will also assist with orientation and precepting of new staff and nursing students as needed.
