Registered Nurse - Habilitation Nurse

We are seeking contract nurses to support the increasing nursing caseloads across the state. The position's duty location will be determined after candidate selection, covering the District East Area. Candidates should be located in this area.

We aim to add five contract nurses to our existing team of approximately 35 full-time District Nurses, who currently support around 1,800 members with skilled nursing needs. The contract nurses will be utilized differently from the current District Nurses, serving as floaters to cover for nurses on leave. Each contract nurse will be assigned to one of the five districts.

Key Functions

• Location varies needed for multiple districts.
• Work hours: 8:30 am - 5:00 pm.
• Travel reimbursement and overtime may be required.

Description of Duties

• Conduct nursing assessments and collaborate with external agencies to ensure appropriate services are delivered.
• Perform in-person visits statewide, serving a core district, monitoring health needs, assisting with referrals, and advocating for members' rights.
• Complete reports documenting the medical necessity for skilled services.
• Conduct in-person assessments every 90 days and as needed; travel is required.
• Complete nursing referrals and place vendor calls.
• Participate in multidisciplinary planning meetings to identify appropriate services.
• Collaborate with members and their support systems.
• Attend mandatory meetings and complete required annual training.

Experience and Expertise

• Registered Nurse licensed to practice, active and in good standing without restrictions.
• Additional training or certification in behavioral health, complex medical conditions, or nursing case management is preferred.
• Minimum two years of recent RN experience in home health, hospice, long-term care, or case management.
• Excellent communication skills, with the ability to explain complex information.
• Proficiency with Google Office, Microsoft Office, and Adobe Acrobat, and a willingness to learn new documentation software.
