Description
We are seeking contract nurses to support the rising nursing caseloads across the state. The position's post of duty will be determined after candidate selection, with a primary focus on the District North Area.
Key Responsibilities
Conduct nursing assessments and collaborate with external agencies to ensure appropriate services are delivered.
Authorize skilled nursing services for members through thorough assessments.
Conduct in-person visits statewide, serving a core district, monitoring health needs, assisting with referrals, and advocating for members' rights.
Complete reports documenting the medical necessity for skilled services accessible to members.
Perform in-person assessments every 90 days and as needed.
Complete nursing referrals and manage vendor communications.
Engage in special projects as required.
Ensure timely documentation as per established guidelines.
Participate in multidisciplinary planning meetings to identify appropriate services.
Collaborate with members and their support systems.
Attend mandatory meetings and complete required annual training.
Qualifications
Registered Nurse licensed to practice in the state, active and in good standing without restrictions.
Preferred additional training or certification in behavioral health, complex medical conditions, or nursing case management.
Minimum of two years of recent experience as a Registered Nurse in home health, hospice, long-term care, or case management.
Excellent communication skills, with the ability to explain complex information effectively.
Proficiency in Google Office, Microsoft Office, and Adobe Acrobat, with a willingness to learn new documentation software.
Additional Information
Location varies, with a requirement for travel to complete in-person assessments. Travel reimbursement and overtime may be applicable. The position operates from 8:30 am to 5:00 pm.
