Registered Nurse (RN) – Home Care

Location: Center City & North Philadelphia, PA (Team 2)
Facility: Bala Cyndwyd PA
Contract Duration: 13 Weeks
Schedule: Monday–Friday | 8:30 AM – 5:00 PM
Guaranteed Hours: 40 Hours/Week
Employment Type: Local Contract (Nursing)

Job Description

Penn Medicine at Home is seeking an experienced Registered Nurse (RN) for a 13-week local contract. The RN will provide skilled nursing care to patients in their homes, coordinate care plans, perform assessments, administer treatments, and educate patients and caregivers to promote optimal health outcomes.

Responsibilities

  • Perform comprehensive nursing assessments in patients' homes.
  • Develop, implement, and update individualized patient care plans.
  • Administer medications, wound care, IV therapy, and other skilled nursing treatments as ordered.
  • Monitor patient progress and communicate changes in condition to the healthcare team.
  • Educate patients and caregivers on disease management, medications, and home care.
  • Maintain accurate and timely documentation in the electronic medical record (EMR).
  • Collaborate with physicians, therapists, social workers, and other healthcare professionals.
  • Ensure compliance with all Penn Medicine, state, and federal regulations.

Requirements

  • Active Registered Nurse (RN) license in Pennsylvania.
  • Minimum 2 years of recent Home Care/Home Health RN experience preferred.
  • Current AHA BLS Certification.
  • Valid driver's license, reliable transportation, and auto insurance.
  • Strong assessment, communication, and documentation skills.
  • Experience with EMR systems preferred.
