    Title: Registered Nurse - Home Care (FT)
    Status: Full-Time, Regular
    Location: Wall, New Jersey ( Monmouth County)
    Salary: Starting at $90,272
    Visa Support: No Visa Sponsorship Available
    Position Overview

    An established home health organization is seeking a compassionate Registered Nurse to provide individualized nursing care to patients in their homes. This role offers the opportunity to build meaningful one-on-one relationships with patients while delivering high-quality clinical care in a home setting. The position features a consistent daytime schedule with no on-call requirement.

    Schedule
    • Full-Time (40 hours per week)
    • Day Shift: 8:30 a.m. – 5:00 p.m.
    • Every fourth weekend
    • Two holidays out of six annually
    • No on-call responsibilities
    Key Responsibilities
    • Deliver skilled nursing care through comprehensive assessment, planning, implementation, and evaluation of patient care in the home.
    • Ensure all care follows physician orders, organizational policies, and regulatory requirements.
    • Perform nursing assessments, medication reconciliation, and collaborate with physicians to develop and update individualized care plans.
    • Maintain timely, accurate, and compliant clinical documentation.
    • Monitor patient progress and revise care plans based on changes in condition.
    • Educate patients and caregivers using individualized teaching strategies to support treatment goals.
    • Coordinate care with physicians and interdisciplinary team members to promote positive patient outcomes.
    • Communicate changes in patient status promptly to appropriate clinical leadership and providers.
    • Maintain compliance with insurance authorizations and payer requirements.
    • Supervise home health aides, complete required supervisory visits, and participate in performance evaluations.
    • Meet productivity expectations while maintaining high standards of patient care.
    • Stay current with evidence-based practice and incorporate updated clinical standards into patient care.
    • Perform additional duties as assigned.
    Required Qualifications
    • Associate Degree in Nursing or Diploma in Nursing.
    • Minimum of six (6) months of Registered Nurse experience or at least one (1) year of Licensed Practical Nurse experience.
    • Home health or hospice experience is preferred but not required.
    • Basic computer proficiency.
    • Ability to lift, push, and pull up to 50 pounds and remain on your feet for extended periods as required.
    Preferred Qualifications
    • Bachelor of Science in Nursing (BSN).
    Required Licensure & Certifications
    • Active New Jersey Registered Nurse (RN) License.
    • American Heart Association Basic Life Support (BLS) for Healthcare Providers certification.
