Registered Nurse (RN) Case Manager – Home Health

Location: Plainsboro & West Windsor, NJ
Position Type:  13 weeks contract 8:00am-4:30pm
Setting: Home Health
Local candidate Required 
Schedule: Full-Time, Day Shift
Coverage Area: Plainsboro and West Windsor, New Jersey

Penn Medicine Home Health Princeton Health is seeking an experienced Registered Nurse (RN) Case Manager to join the South Team serving patients throughout the Plainsboro and West Windsor areas. The RN Case Manager will be responsible for coordinating and managing patient care in the home setting, conducting comprehensive assessments, developing individualized care plans, and ensuring quality outcomes for a diverse patient population.

This position is ideal for candidates who thrive in a fast-paced, high-volume home health environment and are comfortable managing extensive documentation while providing exceptional patient care.

Required Qualifications

  • Active Registered Nurse (RN) license in New Jersey
  • Minimum 2 years of Home Health Nursing experience
  • Previous Case Management experience required
  • Strong OASIS assessment experience required
  • Valid Driver's License and reliable transportation
  • Current BLS Certification
  • Excellent assessment, communication, and organizational skills
  • Ability to work independently and manage a caseload efficiently

Preferred Qualifications

  • Wound Care experience preferred
  • Experience with electronic medical records (EMR) and home health documentation systems
  • Knowledge of Medicare, Medicaid, and home health regulatory requirements
  • Experience managing complex medical and post-acute care patients

Key Responsibilities

  • Conduct comprehensive patient assessments and OASIS evaluations
  • Develop, implement, and update individualized care plans
  • Coordinate care with physicians, therapists, social workers, and interdisciplinary team members
  • Manage patient caseload and provide ongoing case management services
  • Perform skilled nursing visits in patients' homes
  • Monitor patient progress and adjust care plans as needed
  • Provide patient and family education regarding disease management, medications, and treatment plans
  • Complete timely and accurate documentation in accordance with agency and regulatory standards
  • Ensure compliance with Medicare, state, and organizational guidelines
  • Participate in quality improvement initiatives and case conferences

Productivity Expectations

  • Manage a high-volume home health caseload
  • Complete approximately 5–6 patient visits per day
  • Maintain timely and accurate documentation for all patient encounters
  • Effectively balance direct patient care with extensive clinical documentation requirements
