Job SummaryWe are seeking an experienced and compassionate
Home Health Registered Nurse (RN) to join our team and provide high-quality nursing care to patients in the comfort of their own homes. The ideal candidate will be dedicated to patient-centered care, able to work independently, and excel at clinical assessments and care coordination.
Responsibilities - Assess patient needs and develop personalized care plans (e.g., wound care, medication management).
- Provide skilled nursing care consistent with physician orders and agency policies.
- Administer medications, treatments, and therapies as prescribed.
- Monitor and evaluate patient progress and make necessary adjustments to care plans.
- Educate patients and their families on health conditions, medication, and self-care techniques.
- Maintain detailed and accurate patient records and documentation.
- Collaborate with physicians, interdisciplinary care teams, and other health professionals.
Qualifications
- Current and unencumbered Registered Nurse (RN) license in the state of practice.
- CPR/BLS certification (or ability to obtain upon hire).
- Experience in home health care preferred (minimum 1–2 years often preferred).
- Strong communication and organizational skills.
- Ability to work independently, manage a daily schedule, and travel between patient homes.
