Registered Nurse – Home Health Care (Contract)

Location: York, Maine
Contract Length: 13 Weeks
Schedule: Full-Time | 40 Hours/Week | 5 x 8-Hour Shifts
Weekend Rotation Required (2–3 weekends/month)

Position Overview

"Navitas Healthcare, LLC" is seeking an experienced and compassionate Registered Nurse (RN) for a Home Health Care contract assignment in York, Maine. This opportunity is ideal for a skilled nursing professional with prior home health experience and strong knowledge of OASIS assessments.

The RN will provide high-quality patient-centered care in home settings while collaborating closely with interdisciplinary healthcare teams to support patient recovery, independence, and wellness.

Key Responsibilities

• Deliver skilled nursing care to patients in home health environments
• Complete and document OASIS assessments accurately and efficiently
• Monitor patient conditions and report significant changes to the care team
• Administer medications and prescribed treatments
• Educate patients and families on care plans, medications, and health management
• Maintain timely and accurate clinical documentation and timekeeping records

Qualifications

• Active Registered Nurse (RN) license in the State of Maine
• Current BLS/BCLS certification
• Minimum 1 year of recent Home Health RN experience required
• OASIS assessment experience required
• Excellent communication and patient care skills
• Ability to work independently in a home care setting
