Schedule: Full-Time | 40 hours/week | 5 shifts (8 hours each) | Weekend rotation: 2–3 days/month
Position Overview
"Navitas Healthcare, LLC" is seeking a compassionate and experienced Registered Nurse to provide high-quality home health care services to patients in their residences. The role involves conducting comprehensive assessments, performing OASIS visits, coordinating care plans, and ensuring continuity of care in collaboration with interdisciplinary teams.
Required Qualifications
- Education: Graduate of an accredited nursing program
- Licensure: Active Registered Nurse (RN) license in the State of Maine
- Certification: Current Basic Life Support (BLS) certification
- Experience: Minimum of 1 year of Home Health Care experience
Key Responsibilities
- Conduct patient assessments and complete OASIS documentation accurately
- Provide individualized nursing care in home health settings
- Educate patients and caregivers on disease management, medications, and care plans
- Maintain timely and accurate clinical documentation
- Communicate effectively with physicians, care teams, and agency staff
- Follow established clinical protocols, policies, and regulatory standards
