Registered Nurse (Home Health)

Responsibilities:

• Conduct comprehensive patient assessments and develop individualized care plans for home health patients
• Administer medications, wound care, IV therapy, and other skilled nursing treatments in the home setting
• Monitor and evaluate patient health status, documenting changes and communicating with the care team
• Educate patients and family members on disease management, medications, and self-care techniques
• Coordinate with physicians, therapists, and other healthcare professionals to ensure continuity of care
• Maintain accurate and timely clinical documentation in compliance with Medicare/Medicaid and agency standards
• Participate in care conferences and contribute to discharge planning and goal setting

Requirements:

• Active Registered Nurse (RN) license in the state of Ohio
• Minimum 1 year of clinical nursing experience; home health experience preferred
• Strong knowledge of Medicare/Medicaid home health regulations and OASIS documentation
• Valid driver's license and reliable transportation for home visits throughout Columbus, OH
• Excellent clinical assessment, critical thinking, and problem-solving skills
• Strong communication skills with the ability to work independently and as part of a multidisciplinary team
• CPR/BLS certification required
