Responsibilities:- Provide skilled nursing care to patients in their homes, ensuring adherence to care plans.
- Conduct patient assessments and monitor vital signs to evaluate health status.
- Administer medications and treatments as prescribed by physicians.
- Educate patients and families on health management and disease prevention.
- Collaborate with interdisciplinary teams to optimize patient care outcomes.
- Document patient progress and maintain accurate health records.
- Assist with daily living activities to enhance patient comfort and quality of life.
- Participate in ongoing training and professional development opportunities.
Requirements:- Current Registered Nurse (RN) license in the state of Georgia.
- Minimum of 1 year of nursing experience, preferably in home health or community settings.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal skills.
- Ability to work independently and manage time effectively.
- Basic Life Support (BLS) certification required.
- Compassionate demeanor with a patient-centered approach to care.
- Proficient in electronic health record (EHR) systems.
