Position Details
Contingent, per-visit pay structure
Orientation and additional training compensated at a separate hourly rate
Competitive wages
Bi-weekly pay with direct deposit
Responsibilities
Conduct initial and ongoing comprehensive patient assessments, including OASIS evaluations
Complete assessments within 24–48 hours of receiving an initial referral
Develop, implement, and revise patient care plans in collaboration with physicians
Provide skilled nursing treatments as outlined in the plan of care
Educate patients and caregivers using clear, understandable instruction
Document evaluations, treatments, and patient responses accurately and in a timely manner
Participate in discharge planning with staff, physicians, and patients
Attend case conferences and required meetings
Supervise LPNs in accordance with agency policy
Ensure compliance with all Federal, State, Medicare, Medicaid, and Accreditation requirements
Minimum Qualifications
Active Registered Nurse license in good standing in the State of Michigan
Minimum of two (2) years of nursing experience (home health experience preferred)
Knowledge of Medicare/CMS guidelines
Proven experience completing OASIS assessments
Experience with EMR systems (Kinnser preferred)
Reliable transportation, valid Michigan driver’s license, and proof of auto insurance
