Salary: $39.75- $56.11 per hour 

How you'll make an impact in this role:

• Provide direct patient care, including assessments, acute triage, blood draws, wound care, and implementation of individualized care plans in accordance with established clinical standards.

• Coordinate and communicate patient care across the interdisciplinary team, monitor patient status, carry out physician orders, and support discharge planning for seamless care transitions.

• Participate in care plan development, team meetings, and patient/caregiver education to promote positive health outcomes and continuity of care.

• Support patient care coverage and staffing needs as a PRN Field Nurse, including weekday/weekend shifts, after-hours on-call rotation, and travel to participants' homes, long-term care, and assisted living facilities; minimum 4 hours per scheduled shift with additional hours based on census and operational needs.

What minimum qualifications you'll need:

Licensure / Certification / Registration:

• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
• Drivers License credentialed from the Illinois Department of Motor Vehicles (IL-DMV) obtained prior to hire date or job transfer date required.
• Registered Nurse credentialed from the Illinois Board of Nursing obtained prior to hire date or job transfer date required.

Education:

• Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
