Our organization, a leading name in the Healthcare industry, is seeking a passionate and dedicated Permanent Registered Nurse for Home Health. This role is integral to our mission of providing top-quality healthcare in the comfort and familiarity of patients' homes. The successful candidate will be an experienced healthcare professional with a strong background in home health nursing and a firm grasp of the Outcome and Assessment Information Set (OASIS). With your expertise, you will join our team in making a significant difference in our patients' lives, providing them with the highest level of care and compassion.

Responsibilities:

1. Administering medications, providing wound care, and other nursing procedures as per the patient's care plan.
2. Conducting regular patient assessments, including physical, psychosocial, and environmental assessments.
3. Utilizing the Outcome and Assessment Information Set (OASIS) to evaluate patient status, needs, and progress.
4. Collaborating with physicians and other healthcare professionals to develop individualized care plans for patients.
5. Documenting patient care and health status accurately and in a timely manner.
6. Providing education and support to patients and their families regarding care plans, medical conditions, and treatment options.
7. Responding promptly to emergencies and effectively managing critical situations.
8. Ensuring compliance with all state and federal home health regulations.
9. Participating in professional development activities to maintain current knowledge in nursing and home health care.
10. Providing supervision and guidance to home health aides and licensed practical nurses.

Qualifications:

1. Registered Nurse (RN) license in good standing.
2. Minimum of 5 years of nursing experience, with a strong preference for experience in home health care.
3. Proficiency with the Outcome and Assessment Information Set (OASIS).
4. Exceptional clinical judgment and strong decision-making skills.
5. Excellent communication and interpersonal skills, with the ability to interact effectively with patients, families, and healthcare team members.
6. Strong organizational skills, with the ability to manage multiple patients and priorities.
7. Commitment to delivering the highest quality of patient care and maintaining patient confidentiality.
8. Ability to work independently, demonstrating initiative and problem-solving skills.
9. Current CPR certification.
10. Valid driver’s license and reliable transportation for home visits.
