Registered Nurse - Home Health

We are seeking an experienced Registered Nurse (RN) to join our Home Health & Hospice team. This role is ideal for a dependable, self‑directed clinician who thrives in the field and is committed to delivering high‑quality patient care across multiple communities.

Position Highlights

• Weekly Hour Guarantee: 32 hours
• Productivity Expectation: 30 points
• Schedule: Day shift
• On‑Call / Weekend Rotation: Preferred 1 week every 6 weeks
• Mileage Reimbursement: Yes — reimbursed from first patient to last patient

Key Responsibilities

• Deliver comprehensive Home Health and Hospice nursing care in patient homes
• Complete skilled nursing assessments, wound care, medication management, and patient education
• Develop and update individualized care plans in collaboration with the interdisciplinary team
• Document all visits and clinical findings accurately and efficiently
• Maintain productivity expectations while ensuring high‑quality patient outcomes
• Participate in on‑call rotation as assigned

Required Qualifications

• Active Texas RN License
• Home Health Field RN experience (required)
• Strong clinical assessment and critical‑thinking skills
• Ability to work independently in the field and manage a dynamic caseload

Preferred Skills & Attributes

• Dependable and highly organized
• Proficient with OASIS documentation
• Experience with HCHB (Homecare Homebase)
• Strong communication and time‑management abilities
