Functions as a care coordinator for Hospice within the hospice setting. Assesses and identifies patient needs in accordance with the hospice concept of care. Coordinates with interdisciplinary team to provide complete patient/family care and referral. Provides pain management and symptom control. Maintains the continuity of care by ensuring patient needs are met.
EDUCATION/EXPERIENCE REQUIRED:
Requires an Associate's Degree or Diploma in Nursing.
Requires understanding and acceptance of the Hospice concept of care, and a working knowledge of current pain and symptom management.
Requires interpersonal skills to present a positive image of hospice to those whom are referred to the program.
Possess the ability to be flexible in adapting policies and procedures to meet the needs presented by hospice patients and families.
Previous hospice experience preferred.
CERTIFICATIONS/LICENSURES REQUIRED:
Requires current licensure in the State of Michigan as a Registered Nurse.
Hospice certification preferred.
