Position Overview
We are seeking a compassionate Registered Nurse to provide individualized hospice and home-based nursing care to patients throughout the assigned service area. This position allows the RN to focus on patients in their homes while coordinating closely with physicians, caregivers, and an interdisciplinary clinical team.
The RN will assess patient needs, establish and update individualized plans of care, deliver skilled nursing services, educate patients and caregivers, and help ensure continuity of care throughout the patient's course of treatment.
Schedule
- Full-time, 40 hours per week
- Day shift: 8:30 a.m. - 5:00 p.m.
- Weekend requirement: One weekend per month
- Holiday requirement: Two of six holidays
- No on-call requirement
- This position is not eligible for telework
Key Responsibilities
- Perform comprehensive nursing assessments and identify patients' ongoing clinical and supportive care needs.
- Develop, implement, evaluate, and revise individualized nursing plans of care.
- Deliver skilled nursing services safely and in accordance with physician orders, clinical standards, and applicable regulations.
- Complete medication reconciliation and collaborate with physicians regarding patient care plans. Monitor patient progress and ensure visit frequency remains consistent with physician orders and applicable authorizations.
- Maintain accurate and timely clinical documentation of assessments, interventions, education, care coordination, and patient outcomes.
- Apply sound clinical judgment and critical-thinking skills when responding to changes in patient condition.
- Provide patients and caregivers with education tailored to established care goals and individual needs.
- Communicate changes in patient status and clinical concerns promptly to physicians and appropriate nursing leadership.
- Coordinate care with physicians, clinical leadership, and interdisciplinary team members to support appropriate services and referrals.
- Facilitate effective handoffs when patient assignments are transferred or during scheduled time away.
- Maintain required productivity while completing assigned responsibilities accurately and independently.
- Provide clinical supervision to home health aides, including required supervisory visits and participation in staff evaluations.
- Stay current with applicable clinical standards, regulations, and evidence-based nursing practices.
- Perform other related clinical responsibilities as assigned.
- Meet the physical requirements of the position, including the ability to lift, push, and pull at least 50 pounds and stand for extended periods.
Required Qualifications
- Associate Degree in Nursing or Nursing Diploma.
- Active New Jersey Registered Nurse license.
- Current AHA Basic Life Support/Health Care Provider certification.
- At least six months of RN experience OR at least one year of LPN experience.
- Basic computer proficiency.
- Ability to provide patient care independently in a home-based environment.
Preferred Qualifications
- Bachelor of Science in Nursing (BSN).
- Previous home care or hospice nursing experience.
- IV certification.
