Job Summary:

As a Hospice RN, you’ll manage a caseload of patients working as part of a dedicated interdisciplinary team. You’ll assess, plan, and coordinate care while monitoring and adjusting treatment as needed to promote the best possible outcomes.

Duties & Responsibilities:

As a Hospice RN, you’ll manage a caseload of patients working as part of a dedicated interdisciplinary team. You’ll assess, plan, and coordinate care while monitoring and adjusting treatment as needed to promote the best possible outcomes.

Your Day-to-Day Might Include:

• Performing comprehensive assessments and developing personalized care plans
• Coordinating care and referrals across disciplines and specialties
• Monitoring patient progress and adjusting care to achieve optimal outcomes
• Communicating with patients, families, and providers to ensure continuity of care
• Educating patients and caregivers on managing conditions at home
• Documenting clinical notes and updates with accuracy and clarity
• Identifying and addressing opportunities for care improvement

Requirements:

• RI Registered Nurse license (required)
• Bachelor’s Degree in Nursing or related field (strongly preferred)
• At least 1 year of clinical nursing experience (hospice preferred)
• Current CPR certification
• Valid driver’s license and automobile liability insurance
• Excellent written and verbal communication skills
• Strong clinical judgment and problem-solving abilities
