ABOUT THE POSITION

The County of Riverside is currently recruiting for a Registered Nurse I - Per Diem to work in the Office on Aging. Per Diem personnel are distinguished from regular employees in that they receive additional compensation in lieu of County fringe benefits and work assignments are flexibly scheduled or on an as needed basis.

The essential duties of this position includes, but are not limited to, conducting comprehensive nursing assessments and reassessments in clients' homes to evaluate the clients health status and needs, collaborating with social workers and other care team members to develop individualized care plans, implementing services and interventions outlined in the care plan, and monitoring the effectiveness of care plans and adjusting them as client needs change through ongoing home visits. Additional duties include identifying and developing support systems that help clients remain safely in their homes or community settings, coordinating with physicians and other health professionals to ensure continuity of care, maintaining accurate and complete documentation in client records according to agency standards, confirming that all other appropriate community and insurance-based resources have been explored before authorizing additional services, verifying client eligibility and suitability for services during initial screening, and completing required determinations and recertifications related to client care needs on at least an annual basis.

The ideal candidate will have experience working with frail older adults and at least one year of clinical nursing experience or one year of case management experience. The most competitive candidates will have one year of Registered Nurse experience.

Required Licenses & Certifications

  • A current and valid Registered Nurse License to practice as a Registered Nurse in the state of California.
  • A current and valid Basic Life Support, (BLS), certification issued by the American Heart Association.
  • A current valid California Drivers License.

Additional Information

  • Work Schedule: 5/40, Monday - Friday, 8:00 a.m. - 5:00 p.m.
  • Work Location: 3610 Central Ave, Riverside, CA 92506
  • Required to travel by car to clients homes and perform in home visits.
  • Physical Requirements: May be required to walk upstairs in clients homes during home visits.

Office on Aging, one of 52 County departments, is authorized by the Older Americans Act and the Older Californians Act. The department is charged with developing a system of care that offers safety net services for vulnerable older persons and adults with disabilities.

EXAMPLES OF ESSENTIAL DUTIES

Examples of Essential Duties (may include but not limited to)

  • Conduct comprehensive in-home nursing assessments to evaluate participants physical, functional, cognitive, psychosocial, and environmental needs.
  • Develop, implement, and monitor individualized care plans that support participants ability to remain safely in their homes and communities.
  • Complete initial nursing evaluations and reassessments within established program and regulatory timeframes.
  • Provide health education to participants, family members, and caregivers regarding disease management, medication adherence, nutrition, fall prevention, and available community resources.
  • Perform medication reconciliation and assess participants for medication-related concerns and potential adverse effects.
  • Participate in interdisciplinary case conferences and collaborate with social workers, care managers, and community partners to ensure coordinated service delivery.
  • Maintain accurate, timely, and compliant documentation in the electronic health record in accordance with program requirements, County policies, and applicable state and federal regulations.
  • Evaluate home environments for safety risks and recommend modifications or durable medical equipment to support independent living.

MINIMUM QUALIFICATIONS

Education: Successful completion of the coursework required to obtain a valid license to practice as a Registered Nurse in the State of California.

  • License: Must possess and maintain a current valid license to practice as a Registered Nurse in the State of California.

  • Possession of valid Basic Life Support (BLS) cardiopulmonary resuscitation (CPR) and automated external defibrillator (AED) certificates issued by the American Heart Association for professional healthcare providers.

  • Possession of a valid California Driver License may be required for some positions.
