About the Role:ComForCare Home Health Care - Baltimore/Carroll is looking for a compassionate and skilled Registered Nurse (RN) to join our growing home care team in Towson, MD. Responsibilities:- Conduct comprehensive patient assessments and develop individualized care plans
- Administer medications, treatments, and skilled nursing interventions in the home setting
- Monitor and document patient progress, vital signs, and changes in condition
- Coordinate care with physicians, therapists, and other healthcare team members
- Educate patients and family members on disease management, medications, and home safety
- Ensure accurate and timely clinical documentation in compliance with regulatory standards
Requirements:- Active Registered Nurse (RN) license in the state of Maryland
- Minimum 2 years of clinical nursing experience; home health experience preferred
- Strong assessment, critical thinking, and clinical decision-making skills
- Excellent communication and patient education abilities
- Reliable transportation and a valid driver's license
- Proficiency with electronic health records (EHR) and clinical documentation
- CPR/BLS certification required
