Pride Health is hiring a Registered Nurse (RN) Case Manager - Hospice to support our client's healthcare facility in Fall River, MA .

Job Type: Local contract
Contract Length: 13 Weeks
Pay Range: $60.00 to $69.00/hour*
Mileage Reimbursement Rate (if applicable): Standard IRS mileage rate
*Pay offered is based on experience, expertise, credentialing, and education.  
 
Duties: 
Manage a hospice patient caseload and serve as the primary clinical coordinator for assigned patients and families.
Perform comprehensive hospice assessments, routine visits, symptom management visits, and clinical evaluations.
Complete admissions and other case management functions as required.
Collaborate with physicians, social workers, chaplains, hospice aides, and interdisciplinary team members to develop and implement individualized care plans.
Provide pain and symptom management while supporting patients' comfort, dignity, and quality of life.
Educate patients, caregivers, and families regarding disease progression, treatment plans, medications, and available hospice services.
Monitor patient status, identify changes in condition, and communicate updates to the interdisciplinary team.
Maintain accurate, timely, and compliant clinical documentation.

Requirements: 

• Active Registered Nurse (RN) license.
  Minimum recent Hospice Nursing Experience required.
  Prior Case Management Experience required.
  Valid driver's license, reliable transportation, and current auto insurance.
  Strong assessment, communication, and care coordination skills.
  Ability to work independently while effectively managing a field-based caseload.
