Registered Nurse Lead Case Manager

Your future role at a glance:

Location: Tulsa, OK

Facility: Ascension St. John's Medical Center

Department: Case Management

Schedule: Full-Time, Monday-Friday, 8am-4:30pm

How you'll make an impact in this role:

• Mentor Colleagues: Help junior case managers with complex cases and prioritizing their tasks
• Improve Processes: Review case management data to identify trends and enhance workflows
• Prevent Readmissions:​ Develop detailed discharge plans to lower 30-day readmission rates
• Ensure Continuity of Care: Support patients from admission through discharge to prevent care gaps

What minimum qualifications you'll need:

Licensure / Certification / Registration:

• Licensed Registered Nurse credentialed from the Oklahoma Board of Nursing obtained prior to hire date or job transfer date required.
• Case Manager credentialed from the American Case Management Association (ACMA) preferred.
• Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.

Education:

• High school diploma/GED with 2 years of experience, OR Associate's degree, OR Technical degree
  required. Master's degree preferred.

What additional requirements you'll need:

• 5+ years of RN experience
• 3+ years of Case Management experience
• Hospital discharge planning experience preferred
