Position Summary
The Registered Nurse (RN) is responsible for directing and coordinating patient care while providing clinical supervision and support to unit staff in a state-operated hospital or residential facility. The RN assesses patient health needs, develops and implements nursing care plans, administers medications, maintains accurate medical records, and promotes a safe, therapeutic environment. This role collaborates closely with interdisciplinary teams, serves as a patient advocate, and ensures compliance with nursing standards, facility policies, and regulatory requirements.
Essential Duties and Responsibilities
Clinical Care & Treatment
Prioritize, organize, and deliver daily nursing care for assigned patients.
Conduct ongoing assessments of patients' physical and psychosocial status from admission through discharge.
Develop, implement, and revise nursing care plans in collaboration with physicians and other clinical staff.
Respond to emergency situations and appropriately intervene with agitated or distressed patients.
Educate patients and family members regarding medications, procedures, treatment goals, and care plans.
Serve as a patient advocate to promote patient safety, dignity, and quality of care.
Medication Administration & Monitoring
Administer medications and treatments in accordance with physician orders and facility policies and procedures.
Monitor patients for medication effectiveness, side effects, and adverse reactions.
Complete medication administration supervision monitors and clinical observation documentation for support staff.
Coordination, Supervision & Team Support
Assign patient care duties and direct support staff to ensure efficient workflow on the unit.
Communicate patient care issues, safety concerns, and operational challenges to unit supervisors.
Provide orientation, guidance, and ongoing support to staff assigned to the unit.
Collaborate daily with interdisciplinary team members to support coordinated patient care.
Documentation & Compliance
Maintain accurate, timely, and complete nursing documentation, including nursing notes, transfer forms, and other clinical records.
Ensure documentation complies with nursing practice standards, facility policies, and HIPAA requirements.
Adhere to all established nursing, medical, and facility policies and procedures.
Safety, Infection Control & Environment
Ensure compliance with safety, infection control, and environmental standards for the unit.
Utilize appropriate Personal Protective Equipment (PPE) as required.
Maintain vigilance for potentially dangerous situations and respond appropriately to protect patients, staff, and visitors.
General Responsibilities
Demonstrate sound clinical judgment while working with limited supervision.
Maintain professional communication with patients, families, guardians, agency partners, and community stakeholders.
Participate in rotating shifts, evenings, weekends, on-call coverage, and irregular hours as required.
Perform other duties as assigned based on operational needs.
Qualifications
Education & Licensure
Associate Degree in Nursing (ADN) from an accredited institution required.
Current, unrestricted Registered Nurse (RN) license in the State of Indiana.
Current CPR certification required.
Must maintain required behavioral intervention training.
Knowledge, Skills & Abilities
Ability to assess and manage patients with a wide range of medical and psychosocial conditions.
Strong critical thinking, judgment, and problem-solving skills.
Proficiency with electronic medical records, MS Office applications, and clinical documentation systems.
Effective verbal and written communication skills.
Ability to understand and apply medical, nursing, and facility terminology.
Ability to work collaboratively within an interdisciplinary team.
Ability to serve as a patient advocate and promote a safe environment.
Additional knowledge or skills may be required depending on the population served (e.g., children, adolescents, geriatrics).
Supervisory Responsibilities
This position does not have direct supervisory responsibility but provides clinical direction and support to assigned unit staff.
Difficulty of Work
This role requires performing a full range of nursing care for patients with varying medical diagnoses and degrees of illness. Independent judgment is required in applying appropriate nursing care procedures.
Responsibility & Accountability
Work is guided by physician orders and established medical protocols; however, the RN is legally responsible for all nursing actions. Supervision is limited, and work is reviewed for overall adequacy, judgment, and compliance with policy.
Work Relationships
Collaborates daily with interdisciplinary team members.
Communicates nursing principles and care plans to patients, families, guardians, and healthcare representatives in compliance with HIPAA guidelines.
Works closely with support staff to promote safety and operational efficiency.
Partners with agency personnel, public agencies, and community stakeholders as required.
Provides exceptional customer service to agency partners and the general public.
Communicates directly with children and families, as applicable to the facility population.
Physical & Working Conditions
Physical Effort
Ability to run short distances to provide immediate care or respond to emergencies.
Frequent walking, standing, stair climbing, bending, stooping, pushing, and pulling.
Ability to lift and transport up to 50 pounds, with or without assistance.
Above-average strength required for patient restraint when necessary, in accordance with policy.
Requires keen observation and hearing skills to detect potentially dangerous situations.
Working Conditions
Work performed in a state-operated hospital or residential facility.
Potential exposure to infectious diseases, hazardous materials, gas/fumes, dust, noise, and environmental hazards.
Requires high attention to safety due to risk of injury or loss of life.
May require on-call availability, rotating shifts, evenings, weekends, or irregular hours.
Proper use of PPE required.
May experience periods of high stress and pressure.
