About McIntosh Trail

At McIntosh Trail Community Service Board, we offer individuals experiencing symptoms associated with mental illness, addictive disease and/or developmental disability the hope for optimal functioning and recovery by providing quality behavioral health services and supports.

Summary

Under general supervision, assesses client health problems and needs. Develops and implements nursing care plans and maintains medical records. Administers nursing care to ill, injured, convalescent, or disabled clients. May advise clients on health maintenance and disease prevention or provide case management.

Responsibilities

  • Patient Assessment:

o Conduct thorough health assessments of individuals with IDD to identify medical, emotional, and social needs.

o Monitor vital signs, medication administration, and any signs of illness or distress.

  • Care Planning:

o Develop individualized care plans in collaboration with patients, staff, and healthcare teams.

o Set achievable health goals tailored to each patients specific needs and abilities.

  • Clinical Duties:

o Administer medications and treatments as prescribed.

o Perform necessary medical procedures (e.g., wound care) in accordance with protocols.

o Provide education on health management and wellness strategies to patients and staff.

  • Collaboration:

o Work closely with physicians, social workers, and other healthcare professionals to coordinate care and support services.

o Participate in interdisciplinary team meetings to discuss patient progress and adjust care plans as necessary.

  • Advocacy:

o Advocate for the rights and needs of individuals with IDD to ensure they receive proper healthcare and support.

o Educate patients and staff about available resources and services.

  • Documentation:

o Maintain accurate and up-to-date healthcare records, documenting assessments, interventions, and patient responses.

o Ensure compliance with local, state, and federal regulations regarding patient care.

  • Continuous Education:

o Stay current with best practices in IDD care through ongoing education and professional development.

Minimum Qualifications

  • Bachelor's degree in nursing from an accredited college or university OR Associate degree in nursing from an accredited college or university AND current Georgia license as a Registered Professional Nurse AND any related professional work experience.

Physical Demands may include

  • Ability to lift and carry up to 25/50 lbs.
  • Sitting, standing and/or walking, pushing/pulling, handling, reaching, bending, twisting, crawling, kneeling, squatting, climbing (ladders or stairs), overhead reaching.

Associate degree in nursing from an accredited college or university and current Georgia license as a Registered Professional Nurse or Multistate license.
