Responsibilities

• Provide direct patient care for med/surg and rehab patients with timely assessments.

• Administer medications and monitor for adverse reactions with accuracy.

• Participate in performance metrics, targets, and quality improvement initiatives.

• Ensure compliance with safety, infection control, and regulatory standards.

• Develop and update individualized care plans in collaboration with the team.

• Document patient progress and interventions in electronic health records clearly.

• Coordinate with multidisciplinary team to ensure seamless patient transitions.

• Engage in continuous professional development and pursue growth opportunities.

• Educate patients and families on care plans, treatment goals, and discharge instructions.

• Assist in workflow optimization to improve efficiency and patient throughput.

• Mentor junior staff and support a collaborative, merit-based work environment.

• Support travel or local assignment requirements while maintaining high-quality care.

Experience/Qualifications

• Current RN license with med/surg/rehab certification or eligible.

• Proficient med/surg and rehab patient assessment and care planning.

• Accurate medication administration and adverse reaction monitoring.

• Experience with electronic health records and documentation standards.

• Participation in quality improvement and performance metrics tracking.

• Strong infection control, safety compliance, and regulatory knowledge.

• Care coordination with multidisciplinary team for seamless transitions.

• Ability to develop and update individualized care plans with team input.

• Excellent communication skills for patient/family education and discharge planning.

• Willingness to work part-time on-site, travel or local assignments as needed.
