Description

Matrix Providers is hiring a Registered Nurse - Medical Home Port to join our team of talented professionals providing healthcare services to Military Service Members and their families at Naval Branch Health Clinic (NBHC) Mayport.

  • Employment Status: Full -time
  • Compensation: This is an hourly position, paid bi-weekly:
    • $37.34-39.31/hour plus $5.09 H&W
  • Schedule: The NBHC is open from 6:15 am - 6:30 pm Monday through Friday. The Duty Hours are from 7:30 am – 4:30 pm, Monday – Friday. Shift lengths may be variable, from 8.5 hours (to include a 30-minute uncompensated meal break) or 9 hours (to include a 60-minute uncompensated meal break) to ensure shift coverage.
    • Current schedule: 7:30 am - 4:00 pm

Requirements:

  • Education: Possess a Baccalaureate Degree in Nursing, a Graduate Diploma in Nursing, or an Associate Degree in Nursing from an accredited agency such as the Accreditation Commission for Education in Nursing (ACEN) or the Commission on Collegiate Nursing Education (CCNE).
  • Certifications: Must be in person through the American heart Association or American Red Cross
    • Basic Life Support (BLS)
    • Advance Cardiac Life Support (ACLS)
    • Pediatric Advanced Life Support (PALS)
  • Experience: One year of experience in nursing. Having successfully completed at least 12 classroom hours of continuing education within the preceding 24 months that maintains skills and knowledge as an Registered Nurse.
  • Licensure: Possess and maintain a current unrestricted license to practice as a Registered Nurse in any one (1) of the 50 States, the District of Columbia, the Commonwealth of Puerto Rico, Guam or the U.S. Virgin Islands.
  • Provide two (2) letters of recommendation from practicing physicians or peers attesting to the your clinical skills. Reference letters must include name, title, phone number, date of reference, address and signature of the individual providing reference. Letters of reference must have been written within the preceding two (2) years.

Job Summary:

  • Coordinate and deliver comprehensive nursing care in collaboration with interdisciplinary healthcare teams to achieve optimal patient outcomes.
  • Perform patient assessments, physical examinations, health histories, triage, and ongoing monitoring to develop and implement individualized care plans.
  • Administer medications, immunizations, IV infusions, wound care, and other nursing treatments in accordance with provider orders and clinical protocols.
  • Recognize changes in patient condition, respond appropriately to medical emergencies, and provide Basic Life Support (BLS), ACLS, PALS, and other emergency interventions as required.
  • Educate patients and families on health promotion, disease prevention, treatment plans, medications, and home care to support positive health outcomes.
  • Coordinate referrals, specialty appointments, follow-up care, and serve as the Medical Home care coordinator to ensure continuity of care.
  • Supervise, direct, and delegate patient care activities to nursing personnel based on scope of practice and competency while ensuring compliance with nursing standards and MTF policies.
  • Maintain accurate and timely documentation of assessments, treatments, medications, patient responses, care plans, and other required medical records using electronic health systems.
  • Assist providers during examinations and procedures, prepare equipment, utilize clinical information systems, and support the delivery of comprehensive patient care.
  • Participate in quality improvement initiatives, clinical research, preventive health programs, and interdisciplinary collaboration to improve patient care and nursing practice.

Practice Environment:

  • The primary responsibility is managing electronic patient messages for the Family Medicine for two (2) Teams while working in the electronic health record.
  • Duties primarily include reviewing patient messages, preparing prescription renewals and referral requests for physician approval, and handling other electronic communications. Nurses generally do not independently call in prescriptions.
  • Hands-on clinical duties are limited, with only occasional injections or assisting physicians during procedures.

Department onboarding:

  • The onboarding and credentialing process is lengthy. It took approximately six weeks to obtain credentials and computer access, with initial time spent completing computer-based training and shadowing other nurses.
