We Are Hiring:

Location: Fishers, IN

Facility: Ascension Medical Group Clinic 

Department: Fishers Primary Care

Schedule: Monday-Friday 8am-4:30pm

What You Will Do:
  • Ascension Medical Group St. Vincent - Fishers Primary Care 7199 Easy St Fishers, IN 46038
  • Partner with clinicians during Medicare Annual Wellness visits to deliver comprehensive, preventive care for up to 8 patients per day.
  • Advocate for patients by providing clear health education, connecting them to vital community resources, and proactively resolving their care concerns.
  • Execute pre-visit planning and preventive screenings to ensure patients receive timely, accurate follow-up care aligned with wellness guidelines.
  • Manage population health initiatives within ACO guidelines to improve long-term health outcomes for the Medicare patient community.
  • Document clinical interactions with precision, ensuring all services meet CMS and compliance guidelines to maintain high-quality care records.
What You Will Need:

Licensure / Certification / Registration:

  • Registered Nurse credentialed from the Indiana Board of Nursing obtained prior to hire date or job transfer date required.
  • BLS Provider preferred. American Heart Association or American Red Cross accepted.

Education:

  • Diploma from an accredited school/college of nursing OR required professional licensure at time of hire.
Additional Preferences:
  • 8 years of experience in Nursing preferred.
  • Skills: familiar with excel/google sheets, attention to detail, organization, and time management.
