LifeBridge Health is seeking a compassionate and skilled Registered Nurse to join our Healthcare & Medical Services team within our regional health system in Maryland. As a key member of our interdisciplinary care team, you will provide high-quality, patient- and family-centered nursing care across a diverse patient population, supporting our mission to improve health and promote well-being in the communities we serve. In this role, you will perform comprehensive assessments, develop and implement individualized care plans, and closely monitor changes in patient condition in collaboration with physicians and other healthcare professionals. You will administer medications and treatments safely, maintain accurate and timely documentation in the Electronic Health Record, and ensure that all care aligns with current evidence-based practice, regulatory standards, and LifeBridge Health policies. Education is central to this position: you will provide patients and families with clear, compassionate instruction regarding diagnoses, medications, treatment options, and discharge plans, empowering them to manage their health beyond the hospital setting. You will also coordinate care transitions, connect patients with appropriate community and post-acute resources, and contribute to a seamless continuum of care.
