Registered Nurse (RN) Responsibilities:
Participate in the development and implementation of the service plan and initial assessments for clients receiving nursing services to ensure proper placement of staff
Regularly assesses the needs of clients receiving Nursing services
Assess and chart observation of the patient’s condition at each visit
Dress or redress wounds and assess the healing process
Provide education to patients and families on proper home health care procedures and strategies.
Coordinate with Physicians, Physical Therapist, Occupational Therapists, and other individuals in the patient’s care plan
Provide hands on care, (i.e., wound care, ventilator care etc.)
Recommend tools or devices that may improve quality of life for the patients
Regular evaluations and re-evaluations (as warranted by changes in condition but at least every 60 days) of the patient’s nursing needs
Taking verbal orders
Make referrals to other disciplines, as indicated by patient’s assessment
Maintain a current and accurate patient medication profile

Communicate timely and effectively with the agency on changes in visit assignment, dates of scheduled visits, and schedule changes to scheduler and patient care to ensure patient needs are met

Assist clients with their daily functions, such as bathing, dressing, shaving and other activities as needed
Assist with planning for doctor’s appointments or other meetings and may organize transportation for them
Housekeeping, such as vacuuming or doing laundry
Meal prep and the ability to cook meals as needed for the client
Reporting to supervisor about client’s health status

Requirements
Requirements:
Minimum 2 years experience
Basic Health and Tuberculosis screening
Participate in Quality Improvement Program
Able to lift 50 Lbs.
Current RN licensure for State of practice
Current CPR
Ability to think and work independently in a fast-paced and sometimes stressful environment
Valid Driver’s license, vehicle insurance
