Care Coordination Supervisor (RN)

Integrated Behavioral Health & Primary Care

Are you a passionate nursing professional who believes healthcare should treat the whole person, not just the diagnosis? We"re seeking an experienced Care Coordination Supervisor (RN) to help lead an innovative Integrated Behavioral Health & Primary Care program. In this role, you"ll supervise care coordination staff, collaborate with multidisciplinary healthcare teams, and help individuals with behavioral health, substance use, and co-occurring conditions access the care and support they need to thrive.

Position Overview

The Care Coordination Supervisor provides leadership, clinical coordination, and operational support for an integrated healthcare program. Working closely with program leadership, this role oversees care coordination activities, supports interdisciplinary care teams, supervises staff, and strengthens partnerships across healthcare and community systems.

The ideal candidate is a collaborative leader with strong clinical knowledge, experience managing complex client needs, and a passion for improving health outcomes.

What You"ll Do

Leadership & Program Support

  • Partner with leadership to implement and advance integrated healthcare initiatives.
  • Co-lead multidisciplinary teams including behavioral health providers, nurses, medical assistants, case managers, and other healthcare professionals.
  • Support program operations, workflow development, and quality improvement efforts.
  • Help drive a culture of accountability, collaboration, and excellence.

Care Coordination

  • Provide care coordination services to individuals with complex medical and behavioral health needs.
  • Connect clients to primary care, specialty care, behavioral health services, and community resources.
  • Monitor chronic health conditions and support treatment adherence and follow-up care.
  • Conduct health screenings and assessments.
  • Track and document key health indicators, laboratory results, and outcomes within the EMR.

Team Supervision & Development

  • Supervise, mentor, and support care coordination staff, case managers, and medical assistants.
  • Delegate assignments and monitor performance to ensure quality service delivery.
  • Foster professional growth through coaching, training, and ongoing development.

Community & Provider Collaboration

  • Build strong partnerships with healthcare providers, hospitals, and community organizations.
  • Coordinate information sharing and care transitions to ensure continuity of care.
  • Collaborate with external stakeholders and community partners to support program goals and client success.

Quality & Outcomes

  • Promote evidence-based practices and whole-person care models.
  • Utilize data and performance metrics to improve outcomes and service delivery.
  • Support program compliance, reporting requirements, and continuous quality improvement initiatives.

Qualifications

Required

RN Candidates

  • Bachelor"s Degree in Nursing (BSN)
  • Active Registered Nurse (RN) license
  • 1-2 years of experience in behavioral health, integrated care, care coordination, or a related healthcare setting

In Addition

  • Knowledge of integrated and whole-person healthcare models
  • Experience supporting individuals with behavioral health, substance use, and co-occurring conditions
  • Strong clinical judgment, problem-solving, and organizational skills
  • Experience using EMR systems and Microsoft Office
  • Excellent communication and relationship-building skills
  • Commitment to culturally responsive, person-centered care

Preferred

  • Bilingual/Bicultural (Spanish-English)
  • Experience in community behavioral health, primary care, integrated care, or population health
  • Experience working within multidisciplinary healthcare teams

Pay and Benefits

The pay range for this position is $60000.00 - $90000.00/yr.
